Sequoia Transitional Care is seeking a skilled and compassionate Registered Nurse (RN) to join our nursing team as a PM Floor Nurse. This is a full-time, weekday position with some flexibility, ideal for a nurse who thrives in a collaborative, resident-focused environment.
We're offering competitive pay and a $1,500 sign-on bonus for candidates who are ready to make a difference in long-term and transitional care.
🛠️ Your Responsibilities:
- Provide direct nursing care to residents in accordance with individualized care plans and physician orders
- Administer medications and treatments accurately and on time
- Monitor and report changes in resident health or behavior to appropriate team members
- Coordinate with CNAs and interdisciplinary staff to ensure consistent, quality care
- Maintain accurate documentation using facility EMR (PointClickCare preferred)
- Assist with admissions, discharges, and transitions of care
- Respond to emergencies and follow established protocols
- Uphold infection control and safety policies
- Current and active RN license (California)
- CPR certification (required)
- Experience in a skilled nursing or long-term care facility preferred
- Strong clinical judgment, communication, and leadership skills
- EMR experience (PointClickCare is a plus)
