Job Summary: Under the supervision of the In-Home Services (IHS) Director, the manager of Comfort Care and End of Life services oversees the day-to-day operations of the comfort Care/End of Life Program for St. Paul's PACE. The position provides direct supervision of IHS case managers (RN's, LVN's), End of Life Nurses (RNs and LVNs), Home Health Aides (HHA) and department supportive staff. The position provides supervision visits to IHS staff in the field to evaluate skill and performance. The position assists the IHS Director in policy formulation and program development and implementation. This position is responsible for ensuring compliance with annual budgets and financial objectives, company policies and procedures, and guidelines. The position ensures timely completion of participant assessments to meet any policy and regulations. The position acts as the liaison between any contracted hospice agency and St. Paul's PACE and ensures compliance with staff competency requirements. The position maintains the participants' medical record thorough documentation and communicates participants' changes and progress to appropriate staff.

Duties and Responsibilities:

• Oversees the supervision of the IHS End of Life care staff and ensure
  adequate department staffing and staff performance.
• Provide leadership, supervision, and support to clinical staff, fostering a
  culture of professional growth, accountability, and teamwork.
•  Ensures staff perform assessments of participants in their home environment to determine their individual needs and ensures coordinated plan of care with care giving resources.
• Ensures coordination with the Interdisciplinary Team to develop
  a care plan for each participant and that participants are in their least restrictive environment.
•  Functions as a member of the interdisciplinary team. Maintains regular attendance and participates at in interdisciplinary team meetings; communicates participant changes, collaborates on care planning decisions and coordination for ongoing care delivery.
• Identify opportunities for program development and expansion, including new service offerings and partnerships to enhance the scope and impact of end-of life services.
• Completes orientation, initial and annual competencies, supervises and
  evaluates performance of IHS end of life staff.
• Recruit, hire, and onboard qualified clinical staff members, ensuring adequate
  staffing levels to meet patient care needs.
• Perform supervisory visits to staff in the field.
• Ensure staff provide timely notification to the RN Clinic Supervisor, or designee, of any marked change in the participant's condition
• Serves as a liaison between St. Paul's PACE and contracted services.
• Maintains overall quality and fiscal accountability for end-of-life services
  provided.
• Implement quality assurance and performance improvement initiatives to
  monitor and improve clinical outcomes, patient satisfaction, and staff
  efficiency.
• Conduct regular audits and assessments to identify areas for
  improvement and implement corrective actions as needed to maintain
  compliance and quality standards.
• Participates in developing and organizing new staff orientation, training and ongoing in-services training for staff, independent contracts.
• Collaborates with other nursing leaders in the organization and Center Directors on best practices and in end-of-life policies and program development.
• Attends in-service education program, including orientation, skill training and
  continuing education.
•  Receives in-service training in first aid and in cardiopulmonary resuscitation
  within the first six months of employment.
• Follows all St. Paul's PACE Policies and Procedures and OSHA safety
  guidelines.
• Maintains the confidentiality of all company procedures, results, and information about participants, clients or families
•  Practices Universal precautions and follow appropriate Infection Control
  procedures.
• Maintains safe working environment. Follows St. Paul's PACE Care Safety
  Policies and Procedures and Universal Precautions.
• Participates in and supports Quality Improvement initiatives.
• Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.
• Performs other duties as required or requested.

Qualifications and Requirements:

Education: RN degree and CPR certification required. Shall be at least 18 years
of age. Bachelor's degree in Nursing (BSN) or Master's degree in Nursing
(MSN), Healthcare Administration, or related field preferred. HPNA certification
preferred.

Experience: A minimum of two years' experience in working with frail or
elderly population required. Two years of management experience in
hospice or end of life care preferred. Shall have either training or related
experience in the job assigned. 

Skills/Knowledge:

• Strong clinical knowledge and expertise in end-of-life care principles, pain
  management, and symptom control.
• Demonstrated proficiency in strategic planning, clinical budget management,
  and quality improvement processes.
• Knowledge of common safety hazards and precautions to establish a safe
  working environment.
• Skilled in identifying problems and recommending solutions.
• Knowledge of regulatory requirements and accreditation standards related to
  hospice/end of life care, including Medicare Conditions of Participation.
• Skilled in preparing and maintaining records and writing reports, and
  responding to correspondence.
• Excellent organization and documentation skills.
• Familiarity with community resources available to participants and families.
• Skilled in developing and maintaining department quality assurance.
• Excellent interpersonal, communication, and problem-solving skills, with the
  ability to build and maintain effective relationships with diverse stakeholders.
